Pre-Session Rider Intake  ·  Complete Before Your Session

This session is built around you — your riding style, your habits, your gaps. We're going to take you out on the backroads, film you from behind, and watch everything: your body movement, line choice, turn-in points, entry speed, brake markers, how you respond to what the road gives you.

Then we sit down together, go through the video, and talk about what we see — based on what you told us matters most to you. This form is your chance to be honest before we ever get on a bike. Nobody's grading you. The more real you are here, the more useful your session becomes.

About You
Basic info so we know who we're riding with.

Your Bike
What you're riding affects everything we look at on video.
Tires, brakes, chain, fluids. Be honest — this affects your safety on the road.

Your Riding Background
Where you've been shapes where we start.
Crashes, drops, lowsides, highsides — on street or track. No judgment, just context.
Optional, but this is some of the most useful information we can have going into your session.

Your Honest Self-Assessment
Rate yourself 1–5 in each area. 1 = I know I need serious work here. 5 = I feel solid and confident. The gaps between your self-rating and what we see on video are where the real work happens.

Most riders rate themselves a point or two higher than what the video shows. That's not a knock — it's just how awareness works. Be as honest as you can. The more accurate this is, the more targeted your session becomes.

How far ahead are your eyes? Do you look through corners or fixate on what's in front of you?
Posture, weight distribution, how relaxed or tense you are, how you shift in corners.
Entry point, where you clip, exit line, buffer zones, adjusting for debris or oncoming traffic.
Trail braking, brake point selection, front/rear balance, stopping distance, panic stops.
Entry speed, turn-in markers, counter-steering input, committing to the line.
Roll-on timing, smoothness, driving out of a corner, not snapping or chopping the throttle.
Surface conditions, shadows, car behavior, intersections, debris — seeing it before it's a problem.
Keeping pace with a group, traffic pressure, unfamiliar roads, near-misses. How do you respond?

What You Want To Work On
This is the most important section. Your answers here determine the focus of the entire session.
Pick one thing. The most specific you are, the more targeted your session will be.
A type of corner, a situation, a moment where you notice yourself getting uncomfortable.
Something someone's pointed out, or something you notice yourself doing and want to fix.
The flow state. When everything clicked. Describe it if you've had it — or what you're chasing if you haven't.
Something you've wondered about your own riding that you've never been able to see from the outside.

No wrong answer. Just want to know how you define it before we ride together.

Waiver, Release & Confidentiality Agreement
Please read this section in full before signing below. This is a legally binding agreement.

1. Assumption of Risk

I understand and acknowledge that motorcycle riding, including but not limited to street riding, backroad riding, group riding, and skills coaching ("the Activity"), is an inherently dangerous activity that carries a substantial risk of serious bodily injury, permanent disability, paralysis, and death, as well as property damage. These risks include, but are not limited to: loss of control of the motorcycle, collisions with other vehicles, riders, pedestrians, animals, or fixed objects, adverse road or weather conditions, mechanical failure, and the actions of other riders or third parties.

I am voluntarily participating in the Activity, including the Rider Development Session offered by Apex Flow Rider and Vagrant Automotive Racing LLC ("VTR," "we," "us," "our"), with full knowledge of these dangers, and I freely and voluntarily assume all risk of injury, death, or property damage that may result.

2. Release and Waiver of Liability

In consideration of being permitted to participate in the Rider Development Session and related coaching, riding, and filming activities, I, on behalf of myself, my heirs, executors, administrators, assigns, and personal representatives, hereby release, waive, discharge, and covenant not to sue Apex Flow Rider, Vagrant Automotive Racing LLC, its owners, members, employees, contractors, and any other riders or volunteers involved in the session (collectively, "Released Parties") from any and all liability, claims, demands, actions, and causes of action whatsoever arising out of or related to any loss, damage, personal injury, or death that I may sustain while participating in the Activity, whether caused by the negligence of the Released Parties or otherwise, to the fullest extent permitted by law.

This release does not apply to injury or damage caused by the gross negligence, willful misconduct, or intentional acts of the Released Parties, to the extent such a limitation is required by Florida law.

3. My Own Equipment, Vehicle & Insurance

I confirm that I am riding my own motorcycle, that it is properly licensed, registered, and insured as required by Florida law, and that I personally carry valid motorcycle insurance. I understand that VTR and Apex Flow Rider do not provide insurance coverage of any kind for me, my passenger, my motorcycle, or any third party during this session, and that I am solely responsible for any damage to my own property or person.

4. Media Release

I understand that my Rider Development Session may be filmed, photographed, or recorded by VTR / Apex Flow Rider for the purpose of coaching review with me. I grant VTR and Apex Flow Rider permission to use this footage, including clips, photos, and stills, in coaching materials, marketing, social media, advertising, and promotional content, in any media now known or hereafter developed, without compensation to me, unless I specifically opt out below.

5. Confidentiality & Non-Disclosure

I understand that during my session I may be exposed to VTR's and Apex Flow Rider's proprietary coaching methods, techniques, drills, and feedback frameworks ("Coaching Methods"). I agree not to record, reproduce, repackage, teach, or commercially distribute these Coaching Methods without prior written consent from Apex Flow Rider.

I further agree that if other riders, their identities, their riding footage, or any personal information about them is shared with me or becomes known to me in connection with this session, I will keep such information confidential and will not share, post, or distribute it without that rider's consent.

6. Medical Fitness

I confirm that I am physically fit to participate in the Activity and am not aware of any medical condition that would make my participation unreasonably dangerous to myself or others. I agree to inform Apex Flow Rider of any relevant medical conditions or physical limitations prior to my session.

7. Acknowledgment

I have read this Waiver, Release & Confidentiality Agreement in its entirety, I understand its terms, I understand that I am giving up substantial legal rights by signing it, and I sign it freely and voluntarily without any inducement.

Typing your name here serves as your electronic signature on this agreement.
Required if the rider is a minor. Parent/guardian must also agree to all terms above on the rider's behalf.

This waiver is a legally binding document. If you have any questions before signing, please contact us prior to submitting this form rather than after.

Your responses go directly to Apex Flow Rider before your session. We review everything before we ride.
